200 Toyota Tundra Wont Shift Above 3rd Gear

I had my stereo up so did not really hear the engine whining until decel when I could really feel the revs.

When at a full stop sometimes will clunk into lower gear and jolt forward.

Shifts 1st-3rd with out issues wont shift into 4th or 5th driving in 3rd I let off the throttle to coast and the RPM drops to idle level depress the throttle again and goes back up to where it would be.
